COLLEGE ENROLLMENT PROCEDURES For New Students and Transferees:
Step 1: Initial interview with the Program Head and Secure the application form and testing fee slip. Step 2: Accomplished the form and pay admission test fee at the Cashier's Office, after which a signed entrance examination permit will be issued. Step 3: Proceed to the Guidanceand Testing Center for Scheduling and Administration of entrance test. Step 4: Proceed to your respective Program Head to get the result for approval/disapproval to enroll for issuance of plotting form. Step 5: Present the duly accomplished plotting form with the school requirements to the resgistrar's office for Assessment. Step 6: Proceed to the Cahier's Office for payment of Fees. Step 7: Proceed to the Coordinator of Student Affairs Office to secure an ID application and Student Permit Record forms. Be sure that an "ID Apply" stamps is in the registration form. Step 8: Proceed to the Registrar's Office to secure your class cards. Step 9: Report on the first day of class. For Old Students: Step 1: Proceed to your respective Program Head and present duly Accomplished Clearance and Final Exam Permit to the Program Head for issuance of plotting form. Step 2: Present the duly accomplished plotting form with signature of Program Head to the Registrar's Office for Assessment. Step 3: Proceed to the Cashier's Office for Payment of Fees. Step 4: Proceed to the Corrdinators of Student Affairs Office for validation of Identification Card. Step 5: Proceed to the Registrar's Office to secure your class card.
